Athletic Physicals
All student athletes must have a physical examination before they can participate in fall and spring semester athletics. For more information about physicals, please contact Jim Kurowski - jkurowski@bdhs.org, Melody Hopkins - mhopkins@bdhs.org, or nurse Kelly Trojan - ktrojan@bdhs.org.
Physicals are required yearly for any student who participates in Band, Drill
Team, Cheerleading, and/or Sports. Students will not be allowed to participate
until an up to date physical is on file. Please NOTE the changes to this year’s
requirement for physicals.
All students participating in extracurricular activities for the 2008-2009
school year must have had an EKG within the last 2 years in addition to the
physical. The form that is to be completed by a licensed healthcare
practitioner is posted below the TAPPS physical from.
Parents are free to take their child to the practitioner of their choice for physicals. Please have the physician fill out both the TAPPS physical form and the EKG form. Bishop Dunne Catholic School also offers physicals to students on these dates:
EKGs will be offered at both sessions. The cost is $25.00 (for the physical and EKG). Cash or Check only. Please make checks payable to Bishop Dunne Catholic School.
You may sign up early for the EKG portion of the physical. Please email Nurse
